Why has High Speed rail (HS2) gone so badly wrong (over budget and late)?

Part of the issue is that this isn't just about high-speed rail. When you look at the way other countries build things they tend to do the rails separately from the stations. HS2 on the other hand is trying to be all things to all men. For instance old oak common and Euston station are by themselves coming to nearly the original cost estimate for the line, ignoring the costs of Birmingham New Street and Manchester Piccadilly upgrades. The govt could get most of the benefit by building out the tracks but having the final mile run on existing infrastructure. Then each station upgrade could be treated as a separate project.
