GMC register is the cheat code. If the surgeon isn’t on there don’t bother. Centre for Surgery Marylebone was how I ended up finding someone legit, CQC registered so you can literally look them up on the government site before stepping foot in the place

GMC register is your best filter - any legitimate surgeon will be on there and you can verify them directly on the GMC website. Centre for Surgery on Baker Street is one I'd actually trust, they're CQC registered and all their surgeons are GMC listed. Had a consultation with Dr Metin there and the whole thing felt genuinely clinical rather than salesy. World away from some of the others.

Commenting to ask if Scrotox is fake then (dreams shattered)

I've worked on Harley Street for a long time. These are people who get professional referrals and generally don't focus time and effort on websites and digital marketing, so many will look a bit rough.
Harley Street is mostly owned by the De Walden estate, and they are selective in who they allow to practice from there (to keep it's reputation up).
There are a few properties that are not owned by the De Walden estate and these generally tend to house anyone that'll pay - and a billion doctors will work from these few premises; the ones I know for sure are 1, 8, and 10 Harley Street, but there are one or two more I believe.
You can rent a postbox here to say you're a "Harley Street" doctor but you don't even have to practice from their rent-a-rooms, and many I know only hold one clinic there every month or so just to keep up the premise.
Otherwise, you're likely to be fine.

The very vast majority of their business will come via insurance or referrals so direct marketing is often not a focus. Add to the fact a lot are pretty old school and you get the situation you see.
There are unlikely to be that many that are fake, to be honest - but go with something that has a good number of reviews and is referenced elsewhere if possible.
