How is this Azure's fault? A malicious email/document is sent. the end user opens it, provides credentials, and then the phisher/spammer goes to work with the provided credentials. In no way are Azure's backbone servers compromised or anything related to microsoft servers. Title is clickbait. By this logic, ALL sites have been breached which is, in its self, a grandiose super huge, the biggest breach of all.
