Why are CVTs so prone to failure and so expensive to fix?

Disregarding the ways CVTs feel while driving why do CVTs seem to have a bad reputation for reliability and why are they so expensive to fix? Why aren’t they simpler and cheaper than an automatic to maintain and repair? How are they different than a CVT on a golf cart or an ATV which basically last forever?
