I’m talking about the quality of healthcare services and how our public hospitals still look like they’re stuck in the 80s. Sure, they won’t just let you die if you have a serious issue, but I personally don’t trust the so called “free” healthcare system. It’s not about the doctors being bad or anything, it’s more that the hospitals just don’t get enough funding.

Efficient ? No there's not enough surgeons and it gets chaotic.
Good ? Depends on which surgeon you choose.
What do you mean by safety net?
For more serious cases generally even surgeons in Prishtina suggest you go to Nis or Belgrade or Skopje because they're more experienced and better equipped.
But nowdays a lot of Albanians Surgeons with years or decades of experience in Germany/Switzerland are returning home so the level of medical care has also improved.
Unfortunately a lot of them will also choose to work mostly in private clincs which are expensive for the average citizen.

I have no idea, most Albanians do not have private insurance, although a law did pass a week or so ago for state health insurance. I know that for Skopje it's not uncommon for the circle of the person who needs surgery to gather money from other people like when they need to go to France or Turkey for emergent surgery.

One of my cousins' toddler has a cleft nose and they did one surgery to repair that in Belgrade but he needs a second surgery down the line and they're on a big list since it's not urgent, I never asked if they have to pay for it or not.
I know my old math high school professor had a cardiac attack or something that sent him into coma and he spent like a week recovering in Nis, I think it was crowdfunded later on for the sum required during that time.
