Red cross first aid course or any other training of a prep skill.
E.g. you can have all the stuff, but if you can't stop bleeding, it is over either for you or your loved one after stupid accident withou leaving bedroom. And "knowing how to" and "really can and will do" ain't the same thing. This courses are fun, greatly useful and useful to refresh after couple years.
A skill is far better than a thing on a shelf.

Idk the name of the one I have but somehow it is thriving in the worst builder fill-dirt flower bed I have and gives fruit 3X a year. I love it. Really wish I’d planted my blueberries 7 years ago.

Does he have a wish list of hard to find items. Have used World Of Books website to build up a small prepper library. Ebay and Etsy have been useful to find hard to find gifts from wish list items.
If he finds getting out of bed or his chair difficult litter picker grabbers have always been received well from people in similar circumstances.

Depends on the sick part of it, but if he’s in bed a bunch I’ll suggest lock picking tools and some practice locks. It’s an occasionally useful skill and a fun way to occupy your hands if you’re just sitting around.

What about a nice, comprehensive first aid kit? First aid supplies can expire.
My kids say include gloves, gauze, saline, and Superglue. (Kids are nurses, one in an ER)
I like the steri strips that have the plastic cinches on them. Basically it’s to take the place of stitches until the doctor looks at it. Or a nice, basic First Aid book.

Fire extinguishers are more important than almost anything in this list. Other ideas are water leak detectors (to put under sinks, etc.), a car battery jump starter, and a tire inflator to keep in his vehicle.

My mom always gets all the kids a useful prepper type thing for Christmas. This year was fire blankets. I didn’t think I would need it but I was helping put on a demolition derby last weekend and used it on a car engine fire. Probably didn’t save a life but definitely prevented a nasty burn! I ordered three more after that. Super useful and uncommon item we should all have in our vehicles and kitchens!

Because you said fun I would get the freeze dried ice cream sandwiches. I figure in any emergency ice cream is the answer. If you want to make it practical, put them in a nice airlock stage box he can put fabric to avoid dry rot. If you want to make it whimsical make a cardboard box and Saran wrap look like a fire extinguisher box with an "in case of emergency break glass" kind of message. You could say something like "you're the kind of dad that doesn't melt when things get tough"...

Since he isn't feeling great, what about puzzles, cards, board games, dice, download movies or music onto a tablet for him, books, magazines or something for a hobby. Staving off boredom is an important prep and helps the moral while recuperating.

Since he already seems to have quite a bit of equipment, how about a book as a gift instead of more equipment? For example, books on self-sufficiency, medicine and first aid, agriculture, metalworking, DIY, electronics, plumbing, and infrastructure development. There is a lot for a prepper to learn, so there may be books on topics he hasn't yet studied but that might interest him. However, I think the best gift would be to visit him, have a warm conversation, and encourage him to recover from his illness.

Kahtoola has great microspikes. If theres risk of falks from ice during colder parts of the year, highly recommend. Dont buy from Amazon, ebay, etc. They get counterfeited. Buy direct or from an outdoor store like REI. Kahtoola's tend to run small. So go a size up.
Project build kits. If he likes to build some of his own prep gear, buy him a radio kit, so he can build his own NOAA-type radio or something like that. I have some very cool stuff. But some of my favorite things are just stuff I got to build myself, even if they arent as useful as other things I own.

Hold old? Does he have a garden or space to plant trees? I think every prepper wants to be self sustainable. If you get some trees to plant, like avocado, orange, apple, etc They may not bear fruit right away but in a few years they should produce tons of food. If not a lot of space, go with smaller plants like strawberries, tomatoes, jalapeno. Sadly even strawberries haven't worked for me because my yard has tons of aphids and ants who farm them, just destroying tons of plants along with neighborhood racoons/possums/stray cats ravaging our property whenever at night. Maybe rain barrels to collect rainwater. Everyone needs food and water. Bic lighters or 400 packets of honey (amazon $150).
