Sound Fading in/out and sometimes not playing at ALL.

I know why it happens on youtube (Youtube doesn't like add blockers and I subbed to premium lite to resolve that) but it happens for other sources of sound too. I've tried everything from disabling the other sound devices entirely (both in windows settings and in device manager) to updating drivers. I do not know what causes this and I've had this issue with (almost) every regular PC I've had.

have you used the same sound hardware for all of these PCs? like the same headphones or speakers.

Nope, it seems to swap between the speakers and monitor speakers (sometimes some other sound source, which I can't even hear) even though I've disabled these completely, I'd like it to only send sound to the speakers I have plugged into the computer.

Monitor''s are usually samsung, but other than that it's years before I update for a new monitor entirely. This model plugs into the graphics card with one cable if it helps. The other cable is the Power supply.
