I am just curious if he was straightened out enough to make it through in the right lane. Or did he need to be in the left lane because there is a exit he needs to take on the left side. He cant be the only truck going through that tunnel, so do they all have to do that and be fortunate enough that the 4-wheelers are letting him cross over. There has to be more to this maneuver than its the only way they fit.
It’s not a two lane , the lane he starts out in is the oncoming traffic lane. No way this would work out in the U.S. nobody has any interest in being cooperative.
The cars know what's going on, they stay out of the way. They are good drivers in Japan. They know trying to be a Karen will just hold everyone up.
Other vehicles will wait in the tunnel, you can see the mirror when it pans to the tunnel entrance. Japan is so tight that mirrors are standard on corners like these.

The owner several options but the owner chose this option, probably for cost. Or the owner told the engineer to only design for cars or small trucks but then allowed big trucks.
TLDR: your design expertise is no threat to even the most average road/structures engineer.
2 things, look at the space the car behind gives in case another truck comes out, also, he probably has a million worth of cars in that truck so any small bump is going to be expensive.
