It could be an extractor spring issue but with that brig slide, optic and plate, I wonder if there's too much mass to push back on for the round you're shooting. Try a lighter recoil spring or different ammo?
Not a stovepipe, those are vertical, and held inside the chamber by the closing slide before the shell can get out.
What you appear to have going on is a horizontal game of optic handball, where the shell tried to be thrown clear, but then the wall-o-doom optic came right in there and stuffed the dunk.
