Do you apologise if there's no intent in your actions?

you've upset someone or done something wrong but that wasn't your intention or done something accidentally, do you apologise. I've seen that people tend to acknowledge what they've done but feel that an apology is not needed. I feel that regardless of intention an apology should be given.
