Own two, ~700 rounds between them with zero failures. Whats failing? I've seen some of the captured springs break the retainers but otherwise they have been pretty reliable for how many thousands they have sold.

Mine was having issues with the feed ramps not aligning well. I’ve sent it to S&W twice for repair. It did well with 200 rounds of FMJ yesterday. I love the size and concept and I do carry it. I’m just not going to declare it to be a smash hit just yet.

Can you describe the aligment issue you saw - was the barrel ramp overhanging the frame ramp? And did S&W actually acknowledge the problem or just pretend everything was as expected?

The barrel feed ramp overhung the frame ramp so that hollow point, both normal and “external” rounds like the Underwood Xtreme Defender, would become stuck on the barrel feed ramp and therefore a failure to feed. Multiple times per magazine.
The woman I spoke to at S&W customer service was refreshingly knowledgeable and helpful. I very much appreciate the attention to detail and the understanding, escalating my issue for manager review.
Neither the gunsmithing department nor S&W themselves flat acknowledged any sort of manufacturing or tolerance issue, even though I pointed it out to them on the phone and in a letter. They replaced major internals like the barrel and frame parts. They polished the feed ramps and it functions well so far but I haven’t don’t extensive testing. Underwood 380 ammo is pretty expensive.

I appreciate the response. This has always been my suspicion and the reason why I haven't bought one myself. I believe that the 2 piece feed ramp requires greater manufacturing precision and consistency than S&W is able or willing to employ.
The tolerances are probably why we see some people reporting their guns choking on fluted, flat nosed ammo and some HP, while others have no issues feeding those types of rounds. It's a lottery.

I don’t trust it 100%. Frankly I’d feel more comfortable carrying FMJ FN at this point. I don’t think 380 will create much cavitation to speak of, especially in thick clothing or fatty tissue. Over-penetration should always be a concern no matter what rounds are being sent.

Ahh, yeah that's fair. I was worried that the Underwood +P ammo I had wouldn't feed/handle it well, but it did great. My old LCP hasn't been in my pocket since because this is so much more pleasant to shoot while ALSO holding 4 more rounds. The same "design" language of a standard MP makes me laugh though. It looks like I ran a MP through the dryer and it shrunk.
