Applying for passport. Mother’s name is different. What to do?

I will try to make this as brief as possible. Earlier this month I applied for my first adult passport. Took photos. Paid for the application. Sent off documents. I got an email this morning from the Passport Office. My mother’s first name on her birth certificate is different than the one on mine. I was told growing up that the name my grandparents chose was changed to her middle name and she had a new first name when she was 6 weeks old. This would have happened in early 1964 as she was born in December of ‘63. I don’t know if they ever made it official. Both grandparents and mother have all since passed away in the last decade. I have no contact with my father. Not in years. I know my parents got married in September of ‘92 and I was born the following June. But I don’t think they were married for very long as my younger half brother was born in ‘95.
This is my first adult passport. I am at a loss right. I need help.
