I got a speeding ticket for going 73 KPM in a 60 (45 mph in a 37 lmao) at 5 am
What happens if I don't pay it, any tips on appeals or anything like that?

Wait till you try it with hot dogs.

Hotdogs and spaghetti? Sounds like something my kids would eat

Do you live in Italy? If not ignore

Unless you plan on going back. Getting picked up for a ticket would be a shitty way to ruin a holiday.

Was it a rental car? They come after you sometimes and have your credit card on file.

It was in a rental but the credit card I used has since expired

This happened to my dad back in 2004. He was sent 3 follow up pay notices but then nothing further happened.
He even went back to Italy last year fully expecting to be flagged at immigration and made to pay but he got in with no issues.
