Last odometer check reported was 20k miles in 2020. Should I worry about it having sat for long?

Just means it hasn't been professionally serviced or sold since 2020. Might be a DIY car. If privately sold, ask the owner.
If it's for sale by a dealer, there will be no owner to ask, so hard pass.

I do a lot / most of the maintenance myself and keep a spreadsheet / notebook of all the repairs / dates / miles I did the work on. Previous buyers love it and usually I end up “winning” the deal.

Definitely would have some questions and want fresh fluids and tires. But I think that's the gen and trim of the accord that has the same engine as the Type R, so if I were in your shoes I would bite on it and see if you can use the fact that it's been sitting as a way to work down the price.
Realistically, it's a Honda that's under 10 years old, unless it's been sitting in a flood area or some other extremely wet storage place, it's probably fine.
