It looks like one of those fake phishing emails companies send out with a LinkedIn friend request from an attractive women to see how many people click on it and then get mandatory cyber training.

Yeah, it’s legit. SystmOnline is the online patient-facing service for SystmOne, one of the two major GP software providers in the U.K. TPP (tpp-uk.com) is the company that makes it. So that link to upload your ID is valid, even though it looks fishy.
In this case GP at Hand will be using SystmOnline’s backend for providing their service to you.
For context, I’ve worked in uk healthcare for years and have dealt with TPP. Not a great company to work for, going by Glassdoor and what the media has said about the owner.
Any @nhs.net address is likely legit. However, I'd ring the surgery, let them know the context, what you've recieved and ask if its a service they use. Many GP surgeries use some form of online access now (many of which are recent or still rolling out) and there's no set provider they use, but give an ask or look on the specific GP surgery's website and that might give you a better idea. If they've implemented something like this, then they will be able to tell you.
If you were expecting a registration link, it's likely ok
If you weren't I wouldn't click it...
If you're in doubt, call the surgery you registered at and ask if you can provide ID in person rather than digitally.
They should be able to verify and scan it at the surgery (they still cater for the elderly that aren't computer literate, and people that are rightly suspicious of unsolicited messages)

The fact you haven't heard of something doesn't mean it isn't legit. The NHS most definitely does use .net email addresses. NHS.net is the national email/comms platform and has been for decades.
So what if you haven't heard of GP at Hand - have you heard of every GP surgery in the country? They are a genuine GP practice.

Agree it looks awful, but also agree with the consensus that it's legit.
The awful URL: yes, but SystmOne (stylised like that) is indeed one of the main two patient records systems GPs in England use.
The .net address: Yes, the NHS does mostly use NHS.net.
Other things like the stylisation of "Gp" just illustrate their incompetence.
It would be a fabulously brilliant fake to be both bad and good at the same time. Given how right some of the details are, I'm happy it's real.

Whois domain lookup. Search for that and enter the domain name. tpp-uk.com is using private hidden registration. Seems pretty suspicious.

TPP is a service provider that provides a lot of GPs' online systems.
The easiest way to be sure is to call the surgery and ask.
