I'm guessing the trade-offs are cost of fuel vs cost of flight cancellation or having to scramble other equipment? I'm assuming it's better for LH to service the aircraft at FRA?
Does not being able to be at higher cruising altitude affect the ETOPS planning?

There aren’t any passengers on this flight. Does this frame have a damaged fuselage,preventing pressurization? What would the cockpit crew O2 requirements be for an unpressurized oceanic crossing?

ETOPS regulation are only related to commercial (revenue passenger/cargo) carrying flights. Ferry flights (assuming this is one) don't require ETOPS approval.
