It's a silver plated mouse by Wallace silversmiths. There are quite a few of them out there for around $20.

back in the day, Unix users often used three button mice, and it was annoying to get used to two buttons. So, that would be option I'd get. If it had a mouse wheel, it would arguably be less "vintage".
