We go semi-regularly since we have 3 kids and I refuse to subject the nice people at a real steak house to my horrid children.

They do a decent job for a chain, just don't expect the world from them. They can hit medium consistently. Medium rare is a gamble, it probably will be either medium or rare. But they get a great sear on it which is all I'm really looking for.

I worked at Applebees for a while and was trained on the grill (but knew how to cook steak/chicken/etc outside of work) and holy fuck the dudes who normally ran the grill just nuked everything. Someone ordered a medium steak, guy on grill stepped out for a smoke and I temped it. It had long exceeded 200 degrees and was basically jerky

Absolutely looks disgusting. The only way I can imagine this is possible is if someone tried to air fry a steak using a low powered air fryer and set it to like 160 celsius, left it in there for 5 mins and took it out. I wouldn't be surprised if inside is still cold/lukewarm as well. If you did get it at a restaurant (I doubt any cook would serve that, should be fired instantly if they did) write a google review and include the photo to save people from misery
