I personally say when they broke into Stewart's house, vandalized all his family's belongings, and most of all, spilled Stewart's grandma's cremation ashes all over the the place.

Rolling the giant tire (with Beavis inside) caused a trainwreck and a multiple-car accident on the highway. Worst of all, it caused Todd to spill beer on his jeans.

OR....

Butthead shooting into the air, causing that plane to crash to the ground.
