Assisted living facilities Plano/wylie

My mom is a handful of months post stroke. We had to move facilites because of a change in insurance and are at Garnet Hill in Wylie. We need good PT/OT because we want her to come home but we also came to realize we need good nurses because the ones there are just awful to her.
We are looking at Carerra but wanted additional opinions about it and open to anything a few miles from East plano so we can continue to visit and advocate for her. Super bonus if any of the facilities have in house dialysis.

The ones I know of/have had family at or toured are:
PAM rehab hospital in Allen The Belmont in Allen Prestonwood Rehab in Plano
Ask what your insurance will cover/what a doctor can sign you off for to go to. We had to work with a lot of patient services folks to help us understand the different levels of care and what could be covered.
