ULPT Request: Where can get a template or create a receipt for a phone purchase from Verizon or anywhere?

Bought the phone less than a year ago and Verizon doesn't have a record of it. They have a record of my trade in but not the new purchase. Trying not to spend $800 to pay of the remaining on this phone and then buying another one.

If they don’t have a record of it how are they charging you for it?

Right!! They have a record of a purchase, but it doesn't show any details. I've been trying to deal with them for like a month now. Thankfully had a older phone that I plugged the Sim in.
