I would argue that tofu is a perfect food for a malnourished person. When you go without food for too long, it can be very bad to dump a complex meal into the system. Tofu is very light and easy to digest.

Someone who is malnourished needs to slowly reacclimate themself to eating normally. Otherwise, they'll just vomit it back up, which won't help the situation at all. This may be an attempt to slowly ease them into a normal diet, starting with something bland that is unlikely to upset the stomach?

Honestly I'm not surprised. If you are malnourished and eat TOO much you'll die. I'm sure OP would be fine, but there is a reason you don't want someone to gorge themselves so you give them something bland but filling.

I would think the blandness and not being appetising for patients is probably in some way deliberate so that starving malnourished people don’t eat too much too quickly and hurt themselves. Likely why it doesn’t have any meat or much fibre so it’s easier for a stomach to digest food when it’s not used to it

The old every dietary restriction covered in 1 meal trick is great as it simplifies the menu and costs only crushed souls.
5 menu items that are a mix of whatever tastes good. And then the vegan, halal, kosher, dairy/lactose free, nut free, gluten free, low salt, low sugar meal. And if someone ticks any dietary restriction, like piscetarian or vegetarian or no shellfish. Boom - the magic meal of sadness.
I've seen it be a variety of things, from a cup of fruit to much, much worse.

Pathologist here. a healthy person would need to consume somewhere in the range of 500 bananas in one sitting. sounds to me this person likely had underlying health concerns, unless they did in fact eat 500 bananas. so while unlikely, its plausible. id guess an underlying kidney issue was likely at play

He likely did have underlying organ conditions that led to his demise, made acute via.. death by banana. She didn’t go into too much medical detail and believe me, I had to apologize so much because when she brought it up, it was over voice chat.
There was a long moment of silent processing and then I just started busted out laughing because it sounded so ridiculous. But she wasn’t the type to lie or add frivolous details so I fully believe it was the truth. Still, I felt awful and had to keep saying sorry between choking back my laughter. I mean, 50-100 bananas is a lot of potassium. Too little and you can have muscle cramps, chest pain and heart palpitations. Too much and cardiac arrest. It’s key paired with electrolytes in food and drinks, so that in excess… yeah.
I assume each row in a bushel of banana is around 12 or so? So I can only guesstimate how many it would be but still, she told me that was pretty much all he ate and I suppose it built up faster in his system compared to going out. 😬

I had a nurse after my C-section refuse to give me pain meds until I ate something, because it could make me throw up. And refused to let me order food because the food might make me throw up. So basically starving and in pain, my husband was furious. So he ordered food for himself and they let me have broth.
I housed his French fries, threw up a little, and then begged for pain meds. She was VERY reluctant to give them.
The nurse after her was appalled and I had to file a report.

I’m sorry I giggled at this but I do want to say, hopefully as some comfort, that a couple bananas isn’t gonna hurt memaw. If anything they might have needed to balance out what they did to lower her potassium levels - lasix, lokelma, insulin, whatever it was. And grandma probably has underlying health issues like kidney dysfunction or may be on some medication that causes her electrolyte balance to be out of range. Maybe spironolactone, for example. For her K to get high enough to cause cardiac arrest it would have probably been compounded over a while and once the underlying issue is addressed she should be able to enjoy the occasional banana no problem.

I’m a dietitian. The hospital doesn’t keep people inpatient due to BMI. If they are stable they will discharge. Now they may be discharged to a LTC facility or be followed by a related clinic closely, but I’ve seen people with BMIs 9-14 get discharged plenty of times.

from most adult inpatient hospitals in the US, it simply is not possible or practical to keep someone until they are able to gain enough weight to meet those parameters.For the poster to reach a BMI of 15 they would have to gain ~36lbs based on the height they posted, in other words gain over half of what they currently weigh. That does not happen overnight, but ideally in the matter of months or even years of consistent effort.
Not that this is better done at home or anything, definitely whatever is going on likely requires intensive treatment. But at a specialty treatment program/outpatient facility

If you aren't a fan of ensure style drinks that are thick and lactose-like Ensure Clear is the reason I survived radiation and chemo. It's like water and tastes good. (I had stage 3 head and neck cancer in 2022 and i still don't have my taste buds back 100% but the ensure clear has been a lifesaver. A lot of digestive issues due to them. This led to me moving my meals further back so I wouldn't encounter these problems when outside. This caused me to be in a deficit. Once I did eat, I always got sick, so I got scared of food and yeah...guess the fear of diarrhea killed me lol.

My MIL had a lot of problems with diarrhea. She was usually on the “GI easy” diet when she was in the hospital. Supposed to be food that was easy to digest.
One time, they brought her pepper encrusted chicken breast. Like, they had saturated the outside of it in cracked peppercorns. A solid layer.

My aunt has a similar thing after she caught COVID and she's now on a completely vegan & gluten-free diet. It was a tough change, but she was already vegetarian so the transition was easier for her than for some other people. She has a pretty restrictive diet but she's totally back to her old self. She wants to try eating more "normally" again now that it's been a few years, but is understandably pretty anxious about the process and is overall fine with her diet right now.

Autoimmune disease isn’t IBS. It’s an entirely different level of illness that will just kill you if untreated - and treatment is far from perfect, although it’s improved significantly.
You are probably entirely unaware of this, but sufferers often get very angry at suggestions like yours, which are unaware of the scale and severity of autoimmune digestive disorders.
When I first got Crohns, I didn’t eat for almost a year. I ended up in hospital on with IVs as my only nutrition, and then got peritonitis. It’s only because of excellent surgeons that I survived at all.
So please don’t suggest cutting out gluten like this. It’s like suggesting vitamins and exercise to a cancer patient.

hi. i have "IBS" and have been mostly housebound in pain for 3 years. i currently weigh 80lbs and likewise suffer from malnutrition and all the problems that come with it. i recognize i may be a particularly severe case and not representative of the majority of people with this illness, but regardless, IBS is an umbrella/blanket term that covers a wide, WIDE range of symptoms and severity. it's not always just a tummyache, and for some of us there's no treatment that works at all, the idea that my illness could be fixed by cutting out carbs or gluten is equally laughable. please don't throw us under the bus; it's bad enough dealing with the illness on its own without having to see people use me as an example of a "not that bad" illness to point and laugh at, or get offended at being compared to (and the comment you're replying to didn't even mention IBS at all lmao)

I don’t think IBS is laughable. One of my greatest friends has IBS and we often talk about our struggles. I simply gave it as an example because it’s famous, and the similar initials often lead people to believe they are of the same severity, and generally they are not at all.
You may well have IBD - I was told I have IBS due to diet and stress for about a year before I became hospitalized.
But regardless, I do not think IBS is unimportant or easy, and I’m sorry that you felt I did.

I flush alot in public bathrooms to cover the sound. I rock back and forth and count backwards from 10 repeatedly to manage the cramps and blasts. My whole extended family is this way. Fear of diarrhea is valid, I have food fear for the same reasons. Stomach aches and gastrointestinal pyrotechnics. I never got to your level of malnutrition, but it did take me years to realize I shouldn't be able to count the bones in my chest. I quite like some of the flavours tbh. Also they revamped it a couple years ago and it's better now. It's horrible at room temp, needs to be chilled and then it's like a milkshake. I do also recommend making it with milk (or plant-based alternative) instead of water. It's also much better blended rather than shaken, and I usually blend it with some fruit or something. Banana & peanut butter is a reliable one. Sometimes I even blend it with a Snickers and it's like a healthy dessert ¯\(ツ)/¯
Either way I'm not all that sure it will be helpful to OP, I just like to recommend it because it's been helpful for me. I'm NOT a Huel rep/fanboy btw, there may well be other brands that are better. I tried a couple different ones when these things were first popping up about a decade ago, and Huel was waaay more palatable compared to the others at the time - they were literally like drinking cement lol. I stuck with it since and haven't sampled any of the modern competitors.

So sorry you’re struggling like this - I hope they’ve talked to you about issues like refeeding syndrome so that you can continue to stay safe outside of the hospital. You might find recovery material and groups around other restrictive EDs to be helpful, especially because the adjustment period as you get back to a healthy weight can be very uncomfortable and frustrating… it’s always good to have support and people who can reassure you about what’s normal and that it does get better

I have hashimotos, which causes me whole body fatigue/soreness/pain/weakness depending on what I eat. I also have hashimotos. I've never noticed certain foods affecting how i feel though. That's so interesting. For me, I just have autoimmune antibodies constantly attacking my thyroid, killing it bit by bit. Have never had any doctor say anything to me about foods to eat or avoid or anythign though. They just test my thyroid numbers and say that it's still hanging in there and if I talk too much about being tired or not being able to lose the 25 pounds I've put on in the past five years, they tend to ask if I want to see someone for an antidepressant. Which, no I do not. Finally found a NP who would test my free T3 and my T4 and started me on levothyroxine, but nothing improves. It's been like 15 years of samesamesame. Hope you found a better care team to help you through yours than I have mine.

Mine isn't really being managed as I can't afford healthcare unfortunately. It would be misleading to say that there's a diet that will reverse the effects of hashimotos or improve your condition.
Making dietary changes is more about inflammation. With an autoimmune condition, our bodies are constantly experiencing inflammation. That can compound the effects of fatigue and all that comes with it. An anti inflammatory diet can help you feel better. That basically means cutting down refined sugars/carbs and processed foods. Choose whole and healthy foods.
It's pretty hard to do. Eating "clean" requires a lot of discipline and cravings. You don't have to be perfect, but the more you can lean on whole ingredients and healthy stuff the better your body will feel. This honestly applies to everyone, but for auto immune conditions it can be that difference between utter exhaustion for the whole day and feeling a little tired by the afternoon.
I also suggest exercise, specifically strength building exercises like yoga or weight resistance training. It feels horrible to start, but the investment is worth it after just two weeks or so. Even just doing it twice a week, 30 min each session. Total mood and energy boost, which I never would have believed because before when I only tried once I felt drained and weak. But your body craves it and will reward you for it relatively quickly if you give it a chance.

There's probably some correlation, but no direct 1-to-1 translation. You could be morbidly obese and have very little body fat (professional bodybuilders, for example) or dangerously underweight though, very tall and bereft of muscle mass, while having a high body fat percentage.

Malnourished doesn't mean hungry. In fact, many conditions that can cause malnourishment to the point of being on a special diet at the hospital make you less likely to eat even appetizing food, let alone something worthy of this sub. Crackers are inoffensive in smell and texture, unlikely to set of GI issues, and don't look like a single large mass (which as others have detailed is an issue for people struggling to eat). This meal, including the block of tofu, is none of those things...

Of course they might eat it, but there's such a thing is context. They have some issue that makes it hard for them to nourish themselves, and they're posting the food on this sub so they likely find it unappealing. For all of the reasons I detailed, the tofu and this meal in general is unlikely to be eaten by someone in OPs position. Therefore, it's better to focus on giving them food they will be more likely to eat, even with limited protein, rather than a nutritionally complete meal that will stay uneaten.

I mean OP herself is referencing this entire issue is caused by what sounds to be food intolerance so that’s already limited with what the hospital is gonna offer, doubly so if OP doesn’t really know what they should/can eat. The risk for that is around total calories though, not necessarily “heaviness”.
No specific foods increase the risk so you could definitely eat something less shitty while doing slow re feeding.
Being in a hospital and getting your electrolytes monitored is really the most important thing if you’re at risk for Refeeding syndrome anyway.

The mind does wild stuff when you're in a desperate situation like that. That is way too much food to present to someone who knows they can die if they don't eat it and they aren't hungry. It's an entirely different feeling that I'm 100% only 1% of you have ever experienced.
Malnourishment is not the same as being hungry. Anything you can put in your mouth will be bliss if you haven't eaten long enough to become severely malnourished. That's if you can actually get your body to take it in. Having that much on a plate is overwhelming. I know because I've felt that way.

Arfid stands for Avoidant/restrictive food intake disorder.
Usually this consists of having a couple "safe foods" foods you can eat all the time (hopefully) and many unsafe foods. Sometimes it's texture, taste, sometimes it's just how it looks. They have a physical negative response to eating foods they do not like.

Solidarity! I have not needed intervention exactly buuuuut my partner is a physician and he is pretty great at helping me navigate the whole thing with heaps of empathy, so I really lucked out there.

As a former cook this makes me so mad. Every time I see hospital meals o school lunches I feel so angry. I’ve worked in fancy hotel, star restaurant but also in industrial kitchens and government kitchen and we are throwing so much money away in shitty food. Healthy food is not more expensive to produce you just need some qualified people but companies that have public contracts for this kitchen save every penny by industrialising the cooking process and under paying everyone.

Hmm this is interesting. I’ve got an ED and I had a bmi of 10 a few years ago and stayed very far away from doctors because I was afraid they would hospitalise me. I do recall pretty much living a “normal” life, but looking back I can’t have been. I have gained some weight, it’s not near healthy but it’s not a bmi 10 and it’s where I feel comfortable.

I was dizzy for two weeks straight, went to urgent care twice, was sent home. I assumed my dizziness was caused by a new med I was on, but stopping it didn't help. I went to the hospital after contacting a professor who worked with me over a year ago, asking him, if I could do outpatient care with him. I mentioned my weight and height. He forwarded my email to one of the main doctors of the emergency metabolic unit. They offered me a room, said it would be gone in a couple of hours if I didn't accept. I had to agree to an Ng tube. About an hour after walking in to the hospital I died in my room. They wanted to take my rights away, arguing that my brain shouldn't be functioning at this weight. This was a month ago.
Go right away, you can be malnourished at any weight.

I've had the dizzy feeling occasionally, but it typically goes away after having a bite to eat and a rest, and hasn't gotten any worse the past week. The worst part currently is consistent twitching in various muscles, nothing painful, just not particularly comfortable either. Whether or not it's related to malnutrition I don't know, but I haven't found anything about it online and was wondering if you had anything similar?
Unfortunately the doctor/hospital is a good few hours from where I live and it's usually busy and hard to book, but I'll definitely make the effort if things get any worse.

Twitching can be a calcium or magnesium deficiency. I had this in the hospital and made them give me magnesium via IV. This fixed it.

We call it ‘anorexia brain’ a lot of the time but it can happen with any restrictive ED - you lose cognitive functioning and critical thinking goes down. I don’t know if you’ve ever been really starved, but at a certain point, you can literally feel it affect how well your brain is working
Haven’t heard of them trying to take away rights unless the patient is treatment resistant, though, but my experience is mostly with the US/AUS/UK/CAN/NZ systems and OP appears to be in Eastern Europe

The chocolate Soylent is pretty good. Get the powder rather than the shakes of money is tight. If you go with powder, mix it with hot water in a shaker cup, refrigerate, then enjoy later. Gets rid of the powdery texture.

Although, it's more about the flavor than texture- all plant protein, and boy can you tell. Yeah, that could work! Or frozen bananas or peaches, or apple sauce maybe? I made a "pumpkin pie smoothie" from a recipe a couple times, pretty good, think that would work. I try to keep a can of pumpkin on hand for my dogs, so there's usually one open in the fridge.
I'm wondering if using hot water and then chilling will improve the flavor, as well, kinda like how spaghetti sauce or chili get better the next day, flavors blending and calming down.
Plus- pre-mixing the base on days/times when you have the energy and feel okay, and then you've got it ready to go when you don't. Not that it takes a long time, but it's one less step, you know? I have times where getting bread out of the fridge and then toasting and buttering it feels like too much. Never occurred to me, but that's a great solution! I loooove having leftovers in the fridge.

i second soylent!! i remember reading about it when popular science ran an article about it in 2014 or so. the guy who developed it was a broke college student trying to survive as cheaply as possible. it's still pretty cheap today, $40 for 12 bottles, which comes out to less than $4/meal.

With severe malnourishment, you do need to be careful about what you give someone. Too much flavor or spice or variety in one dish can easily upset the stomach or cause shock. It better to start bland and then work up.

When I lived in Europe if a new diabetic patient came to the E.R. they would get continued care in the metabolic unit it's much quieter and more specialized. Every time they were admitted after that for metabolic reasons they went straight to that unit.

Idk if you’re in the ED recovery unit but the food I’ve seen posted from those wards is always vile for some reason. I’ve heard about extra food (usually ensure) even being used as punishment for ‘bad’ behaviours. Mentally ill people are human beings with dignity but psych ward staff seem to forget that sometimes.

Somehow I’m not too surprised with what you’re being fed due to severe malnourishment. Likely they are preventing ‘re-feeding syndrome’ where a malnourished person can get very sick/enter shock due to metabolic and electrolyte disturbances caused by normal everyday food (from the very little electrolytes these people have and their body has acclimated to it). Electrolyte disturbances can cause cardiac failure and arrhythmias.
Ps. I’m sorry but that food does look like a sack of shit, I empathise for you. Praying for your health and happiness x

Okay so I work as a dietitian and this is PAINFUL. Did they supplement thiamine? Did they start you on a liquid diet and how want times have they done a blood draw to monitor electrolytes?? Information: depending on the severity of malnutrition they cannot just feed you dense caloric foods. Tofu makes sense- your body needs easy to digest and process foods as it figures out how to eat food again. If you got something more intense you could develop an intestinal problem.

It may not look good, but I can guarantee you that it’s designed to hit every nutrition metric you need to.
Hospital food shouldn’t be looked at the same as a restaurant. For many people it’s closer to medicine. Balancing specific nutrients, fat, calories, hydration, sodium, etc. is far more important than presentation or taste. You cannot enjoy good tasting food when you’re healthy again.

I was on something like this. Next to no sodium and reduced calories. I was basically starving by a week in. Basically they would have had to put me on the malnourished diet, but I acted all cute and coy and desperate and got one item per day that was off the restricted menu. Still hospital food but that delicious sodium made up for it.

Bright side? tofu is a nutrient dense, easily digestible, soft protein.
Another option? Might be something like Idli... but that's an Indian food (steamed lentil and rice patties) that not many in america would know how to cook. Idli is a convalescent food in the best sense... often given to small children as their first "solid" food... or to elderly with weakened systems... and cooked by steaming, so it's going to be pathogen free as well.

This actually really upsets me. If they are this incompetent they shouldn’t be treating you or any other patients. I work in something similar and we are “overly” cautious about allergies and even makes sure that we don’t serve things the patients don’t like. It’s like the bare minimum to do because it’s THAT important. Hun, if you were in my unit I’d make sure you’d feel safe eating without risking any allergic reactions. This is so dangerous, you should file a complaint if you have the energy to do so.

I was in the hospital once for about a week. The doctor told me I needed to eat more because I was "starving myself". 1. I cannot eat anything sweet it makes me very sick even if its just mildly sweet and 90% of what they brought me was sweet. 2. the not sweet food was absolutely disgusting, when my family could they would visit and bring me food I could eat and i would devour it. 3. I am not thin by any sense of the word I'm 5'1 and 160lbs.
I got so mad one day I looked the Dr right in the face and said "If you take a bite of this and can keep it down I will attempt to eat it" - Shocker he said no he doesn't eat the hospital food because its gross.
