Why do American restaurants take your credit card away?

This is an odd one to me, particularly when tipping. They take your card away to get the details, then bring you a receipt to add your tip and write out the total with a pen and paper, which you then trust them entirely not to edit, and process your payment.
In Europe you either pay at the table or when leaving at a till, and you can add a tip yourself on the terminal. Your credit card is in sight at all times.
Why is America so far behind in terms of this, and how is it not a massive risk for skimming and/or fraud?
NB: This is happening in really good, highly rated restaurants, not dodgy ones.
