Why do chain pub toilets have two doors to get into them ?

I don't understand why, it just causes more of a jam.

....and both doors usually open inwards so that after washing your hands you are forced to use the pull handle that contains all the germs, p*ss and poo from those people who don't! 🤨

To contain odours / spread of germs from getting to the restaurant, noise control (hand dryers, conversations etc), and to create a privacy buffer to prevent direct views into the toilet.

Possibly a regional thing, but in the south east we have a bathroom sink and a kitchen sink

My assumption is that originally a sink was a basin with a plughole.

That brings back memories! We were refused a mortgage on a converted flat in 1983 because the bathroom opened straight off the kitchen.

The pub in the town I grew up in (actually one of four in a small town, but we'll gloss over that) had the men's toilets directly across from the pool table.
It had one door and urinals opposite the door.
Whenever you would get down to play a shot, facing the door, there was the very real prospect of getting an eyeful as the perpetually drunk regulars would exit the loos.

Building Regulations Part T 2.8 put's it a bit more elegantly
"Direct sight lines from a corridor to the WC and washbasin should be avoided, while reducing the number of lobbies where possible (as this restricts access)."

A bar I use occasionally has the lavatory door open directly into the drinking area, and it can be horrendous if someone has dropped the kids off . I don't know how it's got through any planning at all .and the actual door is only 27" wide .

Building regulations used to have a rule that said you had to have two doors in a house, between a bathroom and a kitchen.
Obviously not applicable in a pub, but maybe there is a similar regulation, or used to be?

Those building regs are long gone, as long as the toilet room has hand washing facilities you are fine with one door.

We had a Chinese takeaway locally fail it's council food hygiene scoring for one having one door in the last six months. The owners were not particularly impressed as it's been like it for around 40 years and that was the first inspection that picked up on it...

Privacy? I've been to a few countries (can't think where off the top of my head) where this was the norm. In some it was door > handwashing area > door > bogs.
