Fist is for the more emotional Fuck Yous. When you want the recipient of your fuck you to know it came from the heart. Including the second simultaneous fuck is best in situations of extreme emotion and any interaction with police officers.
Thumb out is the casual fuck you. It's the fuck you , you give to someone who is boring in their annoyance. They are more than a "piss off" but less than the "Fist fuck you"
So the answer is. The correct fuck you is dependent on the situation. Look honestly at the situation and the answer will present itself.

I’m not sure, I think it’s a generation thing. Example A is how I remember my father flipping someone off. He grew up in the 1960’s. Example B (although her thumb is extended out to much) is how my generation was flipping people off in the 1980’s.

The story I heard, which sounds too good to be true, is that during Henry V invasion of France, the French swore to cut off the middle finger of every Englishman once they surrendered so they couldn't be archers any more.

So I realize this is a jokey post, but something you may not know is that the middle finger being a rude gesture originated during the middle ages. If you were warring and your army caught any of the opposing army's archers, you would cut off their middle fingers, because that's the thing that archers used to draw their bow string back with. So by removing their middle finger, you were effectively removing their ability to use a bow with their preferred hand. So sticking up your middle finger at the enemy became either a taunt or a threat, depending on the context - "we're coming for your middle fingers", or "we won so ha-ha we still have our middle fingers", aka "fuck you we hate you".

I was with a close friend in high school and she was driving her 20+ year old rescued from a junkyard car. She was a thrift store rummager, dressing vintage (think Molly Ringwald's "Andie" in Pretty in Pink). Some old guy in a caddy cut her off, so she honked her horn and then pulled up alongside him at the next stoplight. She threw "B" at him.
Old dude just laughed and said, "That's cute, you prissy bitch!"
She turned to me, shocked, then shouted out her window, "Prissy bitch? I'm driving a junkyard Karman Ghia, you dick!" I couldn't stop laughing for about 20 minutes and then just spontaneously broke into giggles the rest of the day.
Of course, after that, her nickname became "P.B.". I even introduced her to my wife at our 20 year reunion as "P.B." which meant retelling the story to everyone at our table who did not go to school with us (giggles were plentiful).

I think it totally depends on the point you're trying to make with the flip! A general FU/ whatever is bent at the middle knuckles. Annoyed is bent at the middle knuckle. "Hello, it's been too long" is bent at the middle knuckle. Anger is a fist. Rage is a double fist. Goodbye is a bent-at-the-middle-knuckle with an air kiss from the tip of the finger.

Funny story about "B". I was with a close friend in high school and she was driving her car. She was a thrift store rummager, dressing vintage like Molly Ringwald in Pretty in Pink. Some old guy in a caddy cut her off, so she honked her horn and then pulled up alongside him at the next stoplight. She threw "B" at him. Old dude just laughed and said, "That's cute, you prissy bitch!" She turned to me, shocked, then shouted our her window, "Prissy bitch? I'm driving a Karman Ghia, you dick!" I couldn't stop laughing for about 20 minutes and then just spontaneously broke into giggles the rest of the day. Of course, after that, her nickname became "P.B.". I even introduced her to my wife at our 20 year reunion as "P.B." which meant retelling the story to everyone at our table.
