I wish I knew. For some reason, I frequently do this at the yard (and I used to do it at truck stops a lot, too). At the yard, I feel like it might be the fact that I put one pump away and there's a second one staring at me, waiting to be put away as well. So I move on to the DEF hose, and forget to cap the diesel tank.
That said, I have no idea how I used to forget at truck stops. I just eventually stopped. I realized I had a problem, started double-checking every time, and somehow just developed a better habit... but I still don't know how I managed to not do it before.

I live in my company's truck, quit spending money on food only buy from Walmart and cook it myself and not have bills. Plus I don't have a wife or kids. Grinding it out, plus I'm the type that all I need is food and a place to sleep everything else is luxury. I'm very simple. My new hobby is to invest in stocks but now I'm not making much money but I'm trying to get my expierence time up so one day I can work for a better company and more $$

Well I have bad news, you won't stop forgetting you can be over a decade of experience and still do it, like me last week. Mind you I made it about 300 ft from the pumps before I stopped and checked and went "awww maaan". For me it's always when I'm trying to be quick and efficient and get out of the guy behind me's way in the pumps.

I have done this one time in 18 years. Was called in for an emergency multi day paving job in February in northern Minnesota. Worked a long last day and didn’t fill my tank before work. I had just under a 1/4th of a tank with about a hour drive to my shop, it was like 5 degrees out with the temp dropping and windy as hell. I filled one tank in a rush before heading home, thank tank didn’t have a chain and I forgot the cap on my steps.
Drove the entire 48 miles back and got out to find the cap still sitting there on the steel steps.

Do a full walk around after fueling. After you put everything up and gloves are off before you pull literally walk around. Will take you 15-20 seconds and if you are looking you should catch stuff like this.

I had this issue, especially the DEF tank, and also forgetting to latch the hood back down. I got into the habit of doing a walk around to check all caps and latches after I pull forward before I walk into the store to get whatever I need. Helps a lot.

You'll remember to close it after your first EPA fine. They like to start around 10k, less than that isn't worth getting involved. I had a friend who drove for FedEx, and drove over a broken pallet someone had dropped on the highway. A piece of it hit his fuel tank just right to rip a hole in it. Luckily, some other trucker passing by saw his predicament and stopped with a spill kit. Putty on the tank, absorbent pads, diatomaceous earth, the works. This happened in California next to a protected wetland area. He was later told if the fuel has gotten off the road surface, or he hadn't taken protective steps that he would have been personally fined 100k and the company would have been fined 1M. His company had spill kits in every truck a week later. A lot of the laws will protect you from fines if you take measures to stop the fuel from leaking. If you drive a sleeper, and don't have a spill kit, sacrifice your bedding. That $100 mattress and whatever your blankets and pillows cost could save you tens of thousands of dollars in fines. Drive safe out there and protect yourselves.

This works. Do it the same way, same order evey time. And if you get interrupted, restart to make sure you didn’t miss anything. Hell, make a written checklist if that’s what it takes. I’m not about to shame a driver for ‘tripping with a checklist. Have a list specifically for fuel stops.

I’ve done this with the DEF cap twice and fuel cap once but luckily some guy saw me spilling fuel that time and yelled at me before I went 10 yards (thank goodness). So now I have this borderline OCD thing about checking all the caps. (When I add coolant, without fail I close the hood and then re-open it to check the cap!)
