I have swapped 7 or 8 banjo housings(the part that actually broke, in international LTs that they use to transfer trash to the landfill. That's been cracked for a while. Someone hasn't been doing proper inspections.

Stress fracture. Pretty rare, but bad suspension system setup makes them happen more often.

More like bad mix of the trailer configuration and the load. Excessive bouncing adds a lot of extra stress. Ideally the suspension does all the movement through a bump and the load never moves. But a bad combination can cause a harmonic amplification of bounce and add stress to the system.
