How does St Luke’s Hospice have my partners details?

Probably a really simple answer, and we can always call tomorrow to check but thought I’d ask here in the meantime in case anyone knows.
My partner has recently lost a relative who received hospice care from St Luke’s. My partner never gave his details to the team, or was really involved in putting the care in to place. However he received a letter today saying they are sorry for his loss, and offering bereavement support.
This seemed a fairly normal procedure to me, until my other half explained he’s never given them his details or spoke to them directly. He has spoken to his relatives who have been involved with the care team, and they have confirmed they haven’t passed his details on, nor have they received a letter themselves (yet).
So has anyone else received anything similar in regards to St Luke’s, or know how they may have got our details?
We don’t mind that they have our contact details btw, we actually thought it was a really nice gesture. Just more curiosity really, and wondering whether someone has to have given them my partner’s details on his behalf?
